New Patient Information

You may register and enter all of your personal and medical information easily and securely. All patients are required to register and utilize the patient portal in order to help us comply with federally regulated HIPPAA requirements. If I need a referral, what happens if I don't have one?
You could pay higher costs.
Review your Summary of Benefits to find out when you need a referral from your primary care physician for the costs to be covered.
If you see a specialist without getting a referral from your primary care doctor, your insurance company will not cover the care you received and you are responsible for all health care costs.
